Request to Remove a User

This form is to be used in the event that a user’s access to your network should be revoked. This request will initiate Fortify’s remove user process. Note that this form may also be used to ‘suspend’ a user while they are on temporary leave from your organization as needed.

Please be advised that Fortify requires at least 24 hours’ notice from the intended removal date for all remove user requests. If this is an urgent and/or time sensitive request and immediate action is required, please call Fortify’s Helpdesk to advise of the urgency. Please note that you will still be required to submit this form in order to finalize your request.
